If your connection disconnects before you have completed this questionnaire
Reconnect after you have completed the questionnaire, then click on the button at the end of the questionnaire. (In other words, you don't have to remain online while you are filling out the questionnaire.)
If you would prefer to keep your connection active, submit your partially-complete questionnaire every few minutes.
If you receive an error message
The most common causes of error messages are:
- Incorrect PID
- Responding too early
- Responding too late
To resolve these problems, please contact the person who assigned your PID.
Page could not be displayed - Encryption
If an error message identifies a problem with your browser, such as "Page could not be displayed", it may mean that you do not have high encryption installed. In this case, you may choose to respond with no encryption. Just click the following link: Respond without encryption.
