To begin building your questionnaire:
- Go to the Questionnaire section within your project
- Start with either:
- Creating questions from scratch (go to Edit Questionnaire)
- Importing an existing questionnaire from the Library or previous project (go to Import Questionnaire)
You can refine and adjust your questionnaire as you go.
Key Steps in Building a Questionnaire from Scratch
Define What You Want to Measure
Start by identifying the areas/competencies you want feedback on, such as:
- Leadership skills
- Communication
- Teamwork
- Role-specific competencies
Clear focus helps ensure your questionnaire is meaningful.
Create Numeric Questions for Each Competency
For each area/competency add numeric questions that reflect the areas you want to measure.
Numeric questions should be short statements that:
- Are clear and easy to understand
- Focus on observable behaviours
- Are relevant to the subject’s role
Define Rating Scale
Choose how responses will be measured.
Examples include:
- Numeric scales (e.g., 1–5, 1–7, or 1–10)
- Agreement scales (e.g., Strongly Disagree to Strongly Agree)
- Frequency scales (e.g., Rarely to Always)
The selected scale should align with your numeric questions. Consistent scales help ensure reliable and comparable results.
Add Narrative Comments
Include open-ended questions to capture written feedback. Typically, an open-ended question is included for each section, as well as at the end of the questionnaire.
These provide:
- Context behind scores
- Specific examples
- Additional insights

Before launching your project, take time to review:
- Question clarity and wording
- Structure and organization
- Consistency of rating scale
- Overall length of the questionnaire

Tips for Building an Effective Questionnaire
- Keep the questionnaire focused and concise
- Avoid overly complex, ambiguous, or duplicate questions
